Buying a percentage of partners home

Good Morning, I'm looking for advice, after a few years of staying at each other's houses, I've finally sold my house to move in with my partner, I'm looking at paying his mortgage off in return for 40% share of his house, we have worked it all out and this is what we have decided after getting his house valued,
Could we do all the legal side of this ourselves or do we need a solicitor?
If we can do it ourselves, could anyone confirm what forms we need?

    I would get a solicitor to do it. Won't cost a huge amount, and may be a godsend in the future. Any conveyancing solicitor should be able to do it , go local.

    You should get a solicitor to complete this for you and at the same time have wills drawn up. Essentially you need to know what will happen if the reltionship breaks up as a result of ill health or death. Some people have a complete personality change in the event of stroke for example. 
    If you are not getting married you should complete a deed of trust to acknowledge percentage ownership.

    there is a guide on the land registry website explaining what forms you need when doing a transfer of equity 
    HM Land Registry forms - GOV.UK (www.gov.uk)

    note however you cannot do it without some cost as you will need the ID 1 form to be signed by a solicitor (and they'll charge for that) or you need to attend a land registry office in person for them to confirm identity.
