Going from WTC with disabilty premium to managed migrated UC but have to stop work as ill health

How can ensure not being worse off transferring from WTC with disabilty premium, working 16 hours to UC, where I am due to be managed migrated to UC but now cannot work due to health issues. Do I change circumstances before or after my claim to UC? What is the best way to not be worse off please? Thank you
