Insurance repair prolonged, and courtesy car. How do I resolve?

Good morning,

my questions are
1) How do I resolve this at no cost to myself if the recommended repairer do not fix the rattle for the second time (eg any courtesy car fees)
2) What are my options with the repairer and insurer and courtesy car need
3) What are the rights I can exercise (I cannot be car less, my job requires me to get to site and I can only do this by car and cannot work from home).
4) What would you do with more knowledge than me in this area.


My car was hit and I went through insurance to repair it via one of their approved repairers. The other driver was liable so I have not had to pay for the repair and courtesy car.  The courtesy car was in my insurance policy too. The damage was to my driver side rear door, wheel both had to be replaced along with some scuffs fixed. There was also some repainting. I have been given a courtesy car while my car was being fixed.
UPDATE: the courtesy car company will not extend the hire as they are saying it is the repairers fault they have not fixed the issue and would like their car back.

My car was returned to me with a rattling sound that comes and goes between 65-70mph. It sounds to me like it is coming from the area of repair. I told the repairer and they came to retrieve my car, but did not fix the rattle which is still there when they returned my vehicle.
 
The garage have agreed to see the car a second time, and suggested I could take it to my car main dealer to get them to diagnose it, and the repairer would pay for this. This to me seems wierd as it would mean another set of hands looking at my car potentially causing another issue.

If the repairer do not fix the rattle for the second time, what are my options for a quick resolution and who pays for it, as I simply cannot drive with the rattling sound. It is mild, but very annoying and was never there, and I drive the motorway every day for work.

I need a car to get to work, so each time the car goes away for a fix, I will need a courtesy car. 
Do I keep a tab of the receipts and costs to myself, eg any taxis back and forth, and who do I bill this to?

I want to avoid a constant back and forth, and with the insurer involved and courtesy car company I am stressed out by these variables. UPDATE: The insurer has informed me the repairer will arrange a courtesy car for me. 

What do I do, as I simply will not accept my car with this new rattle that was not there, and am being severely inconvenienced.
