IHT 406 or 411?

Just in the midst of doing all the IHT forms, and I know the IHT406 is where cash ISAs go, and 411 is where the Stocks and Share ISAs goes, but what about a mixed Investment ISA - the statement shows a cash element with one value, and a stocks and shares element with a seperate value. Just to double check, do I put each element on the relevant forms, or put the combined total on the 411?

I've never heard of a mixed investment ISA. I think the cash is just the cash account associated with the S&S ISA so the total goes on the IHT411.0
