How do you redeem MoneySupermarket Voucher?

It's says I earned £15 for buying car insurance
I can't remember redeeming anything but my wallet says £0
History says I redeemed it? 
I can't see a credit in my bank?

    Did you fill in the claim form within 30 days?
    If so you would have received a gift card or prepaid mastercard
