You state above that you submitted a claimant's questionnaire; did you mean that or did you submit your own that you had received by post from CNBC?

Just send them another copy of your completed DQ within the seven day timescale allowed - i.e. before 26th November.

Make sure it shows you as the Defendant not the Claimant.  Download a fresh one.

And plot your non-available likely weeks right through to the Summer holidays (cross off half terms etc., as unavailable if relevant to you).

I just received a notice of allocation to small claims track and need to write a witness statement... is there a particular template to follow?

Definitely no witness statement template,  each case is bespoke,  so look at recent winning examples within the last 15 months or so
