Just send them another copy of your completed DQ within the seven day timescale allowed - i.e. before 26th November.2

Make sure it shows you as the Defendant not the Claimant. Download a fresh one.

Definitely no witness statement template, each case is bespoke, so look at recent winning examples within the last 15 months or so1
