Grant of Confirmation name issue

Hi all. I've read through the thread and had a bash at submitting a Grant of Confirmation C1 for my deceased mother. It went to Paisley Sheriff court and was sent back with the caveat that the name differs on the C1 to that in the will, which I was fully aware of, but haven't interpreted the guidance notes correctly it would seem.
For the purposes of this post I'll use a pseudonym for my mum's name. She is known to everyone as Joan Gallagher (forename and married name).
She was born Joan Ann Margaret Smith and the name on her will is Joan Ann Margaret Smith or Gallagher.
On page one of the C1 I followed the guidance and gave only her first and second forename ie Joan Ann and surname of Gallagher.
On page 2 I provided her full name as it is on the will. Joan Ann Margaret Smith or Gallagher.
It has been sent back with the highlighted differences in names, and a suggestion that I put an explanation in the declaration to cover this.
Does anyone have a suggestion as to specific wording I should use please?

Here is one option:
Page 2 para 1. enter Joan Ann Margaret Gallagher
Add to para 2 declaration.
In said will the said deceased is named as Joan Ann Margaret Smith or Gallagher, her full legal name including maiden surname.

@buddy9 thank you..Made the change you suggested, popped along with it to Paisley Sheriff Court and it was accepted. Just awaiting the paperwork to come through now. Thank you very, very much with your guidance thrpughout this thread, I managed to complete the form for a large, exempted estate with only one minor issue as above! Thanks!0
