    I found there was a lot of the information I needed for something available through the NHS app: worth a look at and going forward from there.

    Sonething we used to advise clients was to take a pre prepared tick box questionnaire to a GP appointment with the relevant questions where the GP can just tick yes or no and sign at the bottom. It's not appropriate for all circumstances, but for some it can get the information needed. Not all GP's will complete it, but some will happily quickly tick boxes and sign at the end of an appointment for something else, especially if they are very familiar with your history. If they would have to look through your notes to get the info then they either won't do it, or not without a charge. But a tick box report already prepared is far less work than a full report and most GP's want to help, but are limited by their workload. Though this may work better with PIP/Work capability issues than with medical retirement.
