Chargeable Event Certificate

I am trying to complete for my late aunt a HMRC self assessment form and need to include details from a chargeable event certificate that was issued in respect of an investment bond she still held when she passed. After spending over two hours on the phone to HMRC I have come here hoping that some kind person might be able to guide me in the right direction.
I have the information I need to complete the SA100 deceased and think that to register the chargeable event information I will need to use the SA101 form too, the big question is which boxes do I complete, I think it’s boxes 4 and 5, is anyone able to confirm that’s correct? Many thanks
