Capital One Problem

Any advice welcome. I applied and was accepted for an 18 month 0% balance transfer Cap One card. My first statement in may had PPI added to my account which i had not signed up for. I called them and was advised that they would cancel this from my account. The statement for june has arrived and the PPI has been added again.
Can anyone advise on the best way to deal with this. I checked my original application and agreement and at no point did i sign an agreement for PPI.

    Ask to speak to the supervisor/ manager and complain about the PPI being added when not required.
    Tell them it’s not wanted, but has been added twice to your statement!
    Tell them you’re not happy with a company that won’t listen and simply ignores their customers..
    Tell them that you signed an agreement with them! But they are not sticking to their agreement by adding PPI when you never signed for it.
    Ask for a letter saying it won’t happen again and you would like an apology .
