When whoever accompanied your elderly relative asked Nationwide for the complete account details, what grounds did Nationwide give for refusing to supply them?

They didn't refuse to supply, I think printing off a mini statement is what the counter staff automatically do.  Had I been there, I'd have wanted more than that, especially given no statement, account no/sort code or passbook, the latter if it exists being lost.

If it has a roll number then the account number and sort code will likely be one of Nationwide's collection accounts* so probably aren't meaningful, it's the roll number that's important and unique to the account.
