LEAP government scheme for free boiler: Filling in form Problems

Just wanted to bring to people's attention to the challenge re filling in the Self Declaration Form which is part of the application process for the govt funded LEAP scheme. The scheme is to help people on low incomes (you don't have to be on benefits) who have had their gas boiler condemned, or whose boiler has broken and are in a "cold home" situation.
Unfortunately, as I have discovered, the form you have to fill in (and supposedly attach your evidence to) is full of glitches. I have tried several times to fill in my form, but no "attach" or "upload file" button ever appears. Moreover, getting through to the help team is a difficult and long drawn out process, as, I imagine, they are in high demand. The chat team on their website are, unfortunately, not allowed (not trained?) to offer advice.
Thus far, the advice I've been given is that if the right "button" doesn't appear, cancel and start again. But this doesn't appear to be working. I've been in a cold home situation for nigh on four weeks

Is anybody else familiar with this glitch problem?

Update on the LEAP form challenge, for anyone else out there who is struggling with this. Apparently it is only those who are receiving benefits who have to upload evidence at this stage. Applicants who are on a low income, have health problems or who are vulnerable due to age (under 5 or over 65) simply have to submit the form as it is. So once you have added your electronic signature you press the "Yes, I have finished with this form" button and then the submit option appears. It would so much simpler if there were help notes with the form, that made all this clear.0
