Transfer Deed Into Trust Name

Hello
Regarding the death of one tenant in common, after probate has been granted and their share of the property has to be transferred to a trust, what would be the first port of call in doing this?
Is it as simple as going to a solicitor to change the deed?

Are you talking about an immediate post death Trust, set up in a will. Basically leaving half the house to others but allowing the other owner to live there for life, or whatever the will specifies?If you've have not made a mistake, you've made nothing0

Yes i believe this is the case, The other party has life interest in the property.
