UC do bank statements need name/address

Hi all
Just wondering when sending bank statements to UC do they have to have your name and address on them?
Just I'm with Nationwide and its a huge hassle to get them as online it doesn't let you print them with your name and address and can't take screen shots anyway.
Only option is print the statement but it just shows account number, sort code and transactions but no name and address. Is this OK for UC?
Just nearest branch is far away and upto 10 working days for a hard copy sent by post.
