    I would suggest there is no such thing as 'standard' requirements by an insurer.  The OP needs to check the wording of their policy carefully and see if there is any time limit for previous conditions/treatments to be reported.  My current travel insurance states they will autorenew unless asked not to.  Their logic being that their T&C require ANY new medical condition or treatment during the life of the policy to be reported to them.
    A phone call to them can quickly clear up any misunderstanding and remove the risk of cover being denied at a later date.
