How to remove a permanent Caveat on Probate.

A caveat lodged, warning issued, an appearance made, and sealed and made permanent.

All parties have agreed to remove it, I believe a signed consent form will have to be submitted from the parties involved. It was also suggested by another post that a formal summons needs to go with it to the High Court. What is a formal summons. Are there any example forms I can be directed to I do not want to consult a solicitor or a lawyer as the estate is so small. Thank you in advance.
