Obtaining Confirmation on my mothers death

Hi, I was looking for some help. I have completed form C1(2022) to apply for confirmation and took this plus all of the other relevant paperwork to the Sheriff court in Glasgow. The form has been returned referring me to the notes online (which i have read) stating that Box 2 of the declaration has not been completed correctly.

I suspect that i need to add my father in as an executor that is now deceased.  Does anyone know the relevant wording I need to add? Also, do i need to include him on page 1  of the form, even although he is deceased?  Thanks for any help

    Your late father's name should not be included in page 1.

    Are you able to post a redacted copy of what you submitted for box 2.
