My son is classed as having capacity, I would not agree but that is the decision of his mental health team.  I believe he only made this one application.
So would you agree that the best way forward is for him not to make any payments to the card so that he can’t get any more credit?
in the end he will probably have to pay it back somehow but missing a few payments will most likely trash his credit file so he won't be able to get another card

You mention he has a mobile phone...
Not paying can & possibly will lead to not getting a contract in the future. Even sim only over a fixed period can/will lead to a credit check.

Not paying can lead to a world of pain for him going forward.

World of pain? Really? Only those really need mobile contracts, who want very expensive phones that they can't afford. Yet another dangerous temptation for this particular person.
Many people live happily without mobile contracts. There is PAYG and there are auto-renewing bundles with very competitive prices, e.g. at Lebara. I don't remember all details, but one of their selling points is "No Credit Checks".
  • The issue remains that he is an adult and appears legally of capable of entering into a contract.

Has he agreed that you can help him? If so, he could call or go into a branch to explain his situation with you. They may be willing to freeze or remove interest/fees or accept a repayment of the principal sum borrowed.
