Yes I have read the advice and completed the form no problem however I couldn't find anyone mention what needs filling out here? I have a feeling this is meant to be filled out already by others?

For the 'In the' box copy the information from the similar box on your Claim Form.
As for the other box, surely the writing below it gives you your answer.Are you completing the form on behalf of anyone?
Are you 1st Claimant?
Are you 2nd Claimant?
Are you 3rd Claimant?
Indeed, are you any sort of Claimant?
What are you left with?
Once you've worked that out, you'll know what to put in the box.
