HC5 form claim

Hello,

I receive a benefit which means I can claim back costs of travel to hospital. The hospital has closed the Cashier’s Office at the hospital so now I have to claim the long-winded route using a HC5 form. 

You have up to 3 months to make your claim or provide a letter explaining what good reason you have for not claiming within 3 months.

I looked today and it’s 3 months & 5days since my appointment 😡 Does anyone know what they consider as a good reason for being outside the 3 month window?

Any help would be appreciated.

    What is the reason you haven't claimed in time?

    In all honesty - Ill health, coping with mental health problems and avoiding stress, finally looking at form 6 weeks ago and realising I didn’t have an up-to-date benefit letter to send so rang up (took me 4 weeks to summon energy for an hours wait on phone), then took couple of weeks for letter to actually arrive in the post. Bit of avoidance thrown in as I struggle with forms. They’re the gods honest truth reasons but I doubt they’ll be considered acceptable.

    Just write what you've written above?
    Incidentally, depending on which means tested benefit you are claiming (it can't be done for all) you can request a proof of entitlement letter online. It takes about a week for the letter to arrive

    Unfortunately there's no similar link for DLA or PIP, you will need to phone up to get a letter for those.
