Which mistake do I address?

11th May 2022 I got a parking notice for being 11 minutes over my 4 hour paid slot. I was unfit to walk or drive, and had bought chemist painkillers and waited until they worked. I emailed Europarks when I got the notice explaining why I was late and that the fine seemed excessive for that amount of time considering the circumstances and sent a screenshot of the payment to Superdrug from my bank. I got a generic scripted response of it says you'll be charged, if you prove there were no notices send that.
Last week I received a letter from QDR solicitors requesting payment for 5th November 2022 parking charge. I was at the local A&E with my Granddaughter that day so I knew I hadn't. The photo evidence on the security camera says 2022-05-11.
Obviously I'm not paying it, completely happy to go to court. Rang QDR solicitors last week to ask whether I put that I'm disputing the 5th November that didn't happen or the11th May that they ignored my dispute on the forms they sent. No answer so I asked for a callback. Got it today, and he said I can take a payment or put you on a payment plan, I don't know what paperwork you're talking about we just sent a letter. He eventually came back and said put what you think is best within the time frame we've given you (30 days from 19th October)
What do I put on the forms? Everything?

No. The NEWBIES thread tells everyone not to fill in the reply forms.
Why did you ring them? No-one here advises ever ringing a debt crawler chasing a private PCN. Nobody phones QDR. This isn't a reasonable industry! The Government has just declared it in 'market failure'.

I rang them because it's not the date of the charge so I was completely and utterly baffled by it. I don't know what I'm being threatened with legal action for? The charge for 11th May or 5th November. They are insistent it's 5th November.

Your query could've been addressed by email without any risk of giving them any help with their claim.
Are you now following the guidance in the second post of the NEWBIES thread?
11th May or 5th November... that is the question.
Or to put it another way... a simple transcription error... 11/5 or 5/11.
