Advice on Form Cap A5C

Hi I will be grateful if you can help me completing form Cap A5C particularly the section where the applicant needs to swear or affirm the application. The section states 'Insert the names of the applicants, who should swear or affirm if the deceased died before 13 March 1975...' I am applying by myself and it relates to my late mother who died in 1992. The last administrator died without completing the administration my mother's estate.
My question is, do I need to fill this section and take it to a Commissioner of Oaths or leave it blank?
