Restricted licence date confusion

I have my car renewal coming up and I am confused with regard to the question...how long have I held a licence? The information help button says this can be found in the back of the licence.
I past my test in the 1990 and had no problems until recently, I had to inform the DVLA if my diabetes, as a consequence I now have a 3 year licence. There are no restrictions.
The date in the back if the licence is 2022, so do I have to enter that date or the date from 1990?

Alternatively use one of the systems where you can input your driving license number and it collects this sort of information from the DVLA1

On the back of my licence it shows the date I passed my test against the various categories. As I'm over 70 I have a 3 year licence but it still shows the date I passed my test.
