Applying for a blue badge for first time

I have a friend who has received devastating news about their health and now needs to apply for a blue badge urgently. I have shown them the .Gov pages relating to end of life special rules to apply and it says clearly you need a form DS1500 from your GP or Consultant. They have now been told by other people that this form is defunct.
Is it? Even earlier today, the local council Blue badge administrator asked for the DS1500 NOT the SR1 which is required as far as I can see for other benefit applications.
Can anyone help. Has the DS1500 ceased to exist?

Your friend can apply direct, it no longer needs input from a Doctor. It takes a while to complete but can be saved to come back to later. Here is the link to apply: https://www.gov.uk/apply-blue-badge
ETA: I've just gone through the application and cant see a reference to form DS1500, are you perhaps using an out-of-date link?1 -

Could it have been stating that if you have a DS1500, before it was replaced by the SR1, you can use it to apply for things?

I applied on line about 2 years ago, nothing special was required from my just a load of questions to answer
Received it in a few days, I was 80 years old and if that makes a difference I wouldn't know1 -
Got the person concerned to fill in their PIP first and the Blu badge was activated and on its way within 24 hrs!!Amazing.0

It depends on how your conditions affect you and what the criteria is in your LA outside of automatically qualifying.
