What documents do I need to upload when applying for the ULEZ scrappage scheme ?

Hi can any one please tell me what documents do I need to upload when applying for the ULEZ scrappage scheme ?
I understand that you only have a 30 minute window to submit your application and I can't seem to find a list of what is needed to support my application.

Can anyone help please?

Evidence required

To apply, you will need to provide information and upload documents about you and the vehicle you want to apply to the scheme with. You may upload photos, scans or screenshots, including from your phone, but they must show the entire page and be clear enough to read. Files can be uploaded in JPG, PNG, or PDF formats.

About the applicant

You'll need to prove you live in one of the 32 London boroughs or the City of London with a driving licence or recent utility bill/bank statement dated within the last 3 months.

You will also need to provide a form of photo ID showing your name - such as a driving licence, passport or UK Blue Badge.

About the vehicle

You will need to show that this vehicle is registered with the DVLA and that it is insured by providing:

• V5C log book (DVLA registration document)
• Valid vehicle insurance certificate

...and then a lot more about wheelchair adapted vehicles which probably won't apply to you.

Thank you so much.
I am completely over thinking this....and making it out to be harder than it is.
It's the 30 minute time limit that's making me uneasy in case there's a document that is needed that hasn't been scanned.
I shall dive in 2 minutes past midnight ..

Presumably the worst case scenario is just that you have to log in again and start another session?

Needed :-
Passport,
front cover and first page of V5C
Certificate of Motor Insurance
Drivers Licence

Just needed to make a copy of the Reference number on the final page
