TSB Switch - problems on form?

At the end of the application to switch into TSB for the new incentive, you are asked to check boxes to say you have 'read' two documents  - one of these is an account closure form for the old account which appears to want to be filled in, but the fields for sort code, account number etc. won't accept any input (.... on my machine anyway.....)

Tried to phone TSB  - CS rep didn't know so he was putting me through to switcher team .....  but it was 4:48 and they closed at 5:00 so no answer. .....

Anyone else seen this....?

Also,  when asked for the completion date for the switch I mistakenly put in 7 days from today (rather than working days) but it said if you put a date in that was to early it would correct to the earliest possible - however when my partner filled out the form that wording wasn't there....!!

Any ideas ....?

Submitted switch form as part of account application with a proposed switch date 20 days alter.  TSB then sent a message incorrectly claiming the proposed switch date was more than 30 days in the future and therefore cancelled.  Have since attempted to resubmit switch form over a dozen times on laptop and phone with no success.  Have they scuppered their process intentionally?
