Transferring from Vanguard stocks and shares ISA to Halifax cash ISA

As per subject above
I was given a form by Halifax and told to send it to Vanguard but Vanguard seem to be waiting for a request from Halifax and say:
We can receive transfer requests via the following methods:
- Origo, the electronic transfer system used by providers
- Post to: Vanguard, PO Box 24095,1 Tanfield, Edinburgh, EH3 1FS
- Email to: VanguardTransfers@fnz.co.uk
I'm puzzled!
I was given a form by Halifax and told to send it to Vanguard but Vanguard seem to be waiting for a request from Halifax and say:

Halifax need to handle the transfer. You just need to open an account and fill in necessary transfer forms.0

This is the transfer process:Halifax should take care of it. If posting your form to Vanguard did not work, get a clear statement from Vanguard on what they need from Halifax. Use that to chase Halifax.
