    Follow precisely the requirements of their appeals process. Doing otherwise will see them ignoring the appeal, then try to time you out when you chase some weeks later.

    so the appeal form needs salutations names and addresses, do i just put

    Mr "Company" and company address?

    What if i select an category (e.g "Medical: I was attending to an infant/young child in the vehicle" and then state in the appeal message that as there is no category that covers the fact they are following POFA i have selected the last option available as there is no "Other: please explain" category?

    something Like this:-

    Please note that this appeal from on your website does not cover a suitable category for this particular appeal, therefore the last available category option was selected "Medical: I was attending to an infant/young child in the vehicle".
