Can't even obtain a PIP Form!

Hi, I went to my local advocacy agent to help me apply for PIP.
She said I phone a number and then they send me a form, and then I take the form into her and she will ask me stuff and fill it in for me.
So I phoned. And was on the phone for over 45 minutes on the automated system.
I tried again and could not get through. So I went on the website and it said you can write to them instead, to get the form. I did this. And I have heard nothing a month later. And I sent the letter recorded delivery.
I also understood it to mean I was only sending a letter asking them to send me the form. I gave them all the info they asked for. This sounds like a very long-winded way of trying to apply for PIP, especially as it has taken so long so far.
If I were to phone them again and get through, would they be taking me through the form on the phone?
Because this cannot happen!
I have ADHD and dyslexia and despite having an art degree, long forms finish me off. I always mess them up.
I have done it before, and my advocate's sister has the same conditions as me, so she knows what she is doing. She has to fill it in for me.
So if possible, please can someone tell me the best way to get a PIP form?

Or do they take you through the form on the phone?

No just the form and a few questions, they will then send the form out and you can get help with it.1

If your PIP claim is successful your award will be backdated to to date you ordered the form:
