    Do not mention the visa issue to your insurer: as a general rule it is impossible to insure against the acts of governments.

    Note that there are plenty of visa appointments but you need to go through a visa agent (and pay a hefty fee) in order to get them. Were it not for your operation that is what I would suggest that you explore.

    Some  annual travel policies have an obligation to update on any changes. 
    I suspect this may depend on the timing of when you booked the holiday. Did you book the holiday after you knew about your heart condition?

    Not to say that you are not, but perhaps best to take a conciliatory approach with the travel organiser/airline, rather than one demanding (non applicable) ‘rights’.  Your problem to resolve, not theirs.

    I suspect the date on which the cardiac diagnosis was made is a key aspect - highly unlikely a GP would then green light foreign travel with a heart problem........and that medical condition should have been flagged up to the insurance company without delay, otherwise the policy could be rendered void.
    In my experience, elective surgery before travelling is not grounds to claim on a travel policy.
    I was awaiting a gall bladder removal and opted to accept a short notice cancellation thereby nullifying any claim - just had to suck up the loss.
    Health comes first.
