In Chrome I had to open an incognito window to see the form. It should be just about the T&C

Make sure you don't have any adblockers running. I always use a different browser with no adblockers for sites where the forms don't show up when using an adblocker

I managed to enter using FF and private window and the form popped up

I opened in secret mode AND accepted all cookies. Then finally the form popped up
