Esa/Capability For Work Questionnaire

Made a application for New style ESA on 7th April. I was told my the work coach ounce I have handed in a few sicknotes and it's been atleast 13 weeks I will get a capability for work questionnaire to see which group I will be put in. 

This morning I have received in the post the capability for work questionnaire is this normal receiving it so quickly in a matter of a month from applying for ESA

Also ounce the form is sent off will it be a face to face assessment or over the phone? I'd rather have it over the phone due to my anxiety mental health

    That is normal. The original intention was that the WCA questionnaire would be issued immediately and returned will the assessment to make place and a decision make on which group you qualify with within that initial 13 week period.
    Information I post is for England unless otherwise stated. Some rules may be different in other parts of UK.

    Is there only one WCA capability assessment needed or will I need further ones in the future?

Future reviews and initial assessment can include face to face or telephone assessments but sometimes they can carry out paper based such if sufficient information. You could always add additional information to explain how you'd find a face to assessment difficult to attend - I historically have for ESA and PIP and they've never listened to me until now that the last two ended up curtailed due to distress.

    I currently have mental health PTSD hearing aids a non healing fracture and GP suspects Fibromyalgia so has referred me to Reumatolgy I need to send my form back by 7th May it would of been nice if I could of got something from the Rehmstolgist as supporting evidence. But any way I will put on my form for a phone assessment as I am really going to struggle face to face due to anxiety.

    The majority of assessments are by telephone anyway. You could be waiting quite a few months for an assessment, if it's needed but most people have them.

    Put down about your symptoms anyway even if you're awaiting diagnosis/confirmation.  You can always send in more evidence later, but obviously the symptoms are having an effect on you now even if you don't have an official label for them.

    To clarify there are two (qualifying) categorisations of disability - limited capability for work (LCW), limited capability for work related activity (LCWRA). The latter is the more severe classification.

    To qualify for LCW you need 15 or more points in total - only one descriptor for each activity can be applied.
    To qualify for LCWRA you need to meet the criteria for one or more LCWRA descriptor.
