PPI Tax Refund

Tonight I received a phone call from national PPI advice ltd.
They said I was entitled to a tax refund on previous PPI and received. They didn't ask for any bank details or any other details. They said all I have to do was sign an online form which they would email to me and they would do the rest.
I received the email and read it through. The first part was all legalese basically outlining who was claiming and who the agent was with the full name and address. The second part was an R40(M) form attached with the company details completed. The only details referring to me on the form were my name and street name but no house number.
Give me one more details on the form pertaining me I digitally signed the form and send it back.
The company then called me back and said I would need to provide my national Insurance number in order for them to complete the R40(M) form so they could act on my behalf. I advised that I was very dubious and would check with my bank first whether or not this phone was legitimate. They were happy for me to do that and asked me to email my national insurance number onceI had confirmed with my bank.
I rang my bank, who sadly weren't too helpful and more or less left the onus on to verify the stairs of the company.
I've checked them out online and they appear to be registered with Companies House but as far as I can tell, aren't a very big concern.
I'm still very dubious, so thought I would ask you kindly folks on here if any of you have had any dealings with the company.

Any help/advice on whether to proceed with this claim would be appreciated.

Give me one more details on the form pertaining me I digitally signed the form and send it back.

Out of interest how did they know the statutory interest details to include on the R40 form?
And all your other taxable income?0 -

An R40 is a simplified tax return. You may only need to complete a handful of boxes.
Just don't forget PPI refunds aren't taxable and do not have tax deducted from them.
The statutory interest paid alongside the PPI refund is taxable income and does have tax deducted.
