Hi, I’m wanting to reclaim my PPI tax by completing an R40 form. 

Can anyone help as I’m not sure what amounts to put in the boxes! (3.1, 3.2, 3.3). 

This is my letter I received, which amounts go in which boxes?

The gross interest is £580

The tax deducted is £116

The net interest is the gross amount less the tax.

You don't include the PPI refund itself on the R40 as that isn't taxable income.
