Child benefit person receiving change form

Hi. I've been on the phone to child benefit and they said they would send me a form out a while ago but haven't received it.

My daughter's mother is wanting the child benefit payment and I've agreed. Which form would she complete / would have been sent out for her to complete? as I'll just get it printed off instead if anyone knows?

  I'd suggest your daughter's mother completes the CB claim form, and enclose a signed note from yourself stating that you wish to relinquish your claim in favour of hers.
