Depending on whether her GP has said she cannot fly due to medical issues they may be able to claim on their travel insurance.
Pregnancy and choosing not to travel isn't usually covered but check their terms because many do cover if there is a medical reason not to travel during pregnancy0 -

Check the T&C of their travel insurance. They may be able to claim on that. I'm assuming they took out travel insurance when they booked the holiday.

The other issue will be... if they dont cancel now can they still get a refund of the hotel closer to the time? If not it may be a gamble that "hopefully" they get ill so they can claim on the insurance and the insurance pays out rather than cancelling now and getting the hotel cost back.0 -
