Attendance Allowance

Hi all, I'm helping mum fill out the attendance allowance forms for my dad, mum holds power of attorney already so does she need to tick the 'i want to be appointed to act on their behalf' box ? Ive looked on the gov website and it doesn't say. 
Also I'm presuming that you can put additional info on a separate piece of paper as they don't give enough room to write everything down ? 
Its quite difficult as my dad has not been diagnosed with Alzheimers/Dementia yet (is in process of)

    Yes, tick the box. By doing this the process will start for either yourself or your mum to become his appointee. You will then be responsible for reporting changes and filling out forms, as well as all phone calls.

    I went through all this a few years ago for both my parents.
    I think the AA forms were 85 pages long if I remember correctly !
    Tick the ' I want to be appointed ' box.
    You Mum should sign the form as Attorney.
    Look up on Google it tells you how to sign as Attorney.
    Can't just recall the exact words & in what order as both my parents passed away earlier this year 
    You can certainly use extra sheets for your information.
    Make sure you list every single tiny little thing that your dad needs help with.
    Lastly keep a copy of everything you do.

    Are either you or your mum able to claim Carers Allowance for your dad ??
