Transplant recipient, PIP

Helping a friend, and have 2 questions.
1. She was in complete kidney failure (diabetes) and on Enhanced/Enhanced PIP and SG ESA.
Went in for transplant op, but there were severe complications. She ended up having 4 major ops, lost the donated pancreas, but is now home with functioning kidney (YAY!)
She was supposed to be out of hospital within 28 days, but ended up staying in for over 40. Nobody informed PIP because OMG will she survive?
Now she needs to inform PIP but is not exactly compos mentis (major pain meds), so I'm taking over.
She will need to repay her mobility for the extra days, yes? How do I inform them, and how do I counter any "Why weren't we informed sooner?" questions?
2. Obviously her health has changed. Both PIP and ESA should probably be informed, right? She won't be capable of anything more than taking meds and physio for at least 6 months. When, and how, do we inform them?

Sorry I can't help with question 1, but question 2 - unless her health has significantly improved, there is no point contacting PIP or ESA about a change because she already has the highest awards possible.
Worsening health won't result in a different award, so telling them her health has changed will just lead to unnecessary reassessments where the best possible outcome is her benefits stay the same, but with the stress of filling out the forms and new assessments. (The potential worst outcome is they make a totally inappropriate decision which results in her benefits stopping, so she then has the stress of having to fight for the right decision which could take a year or more AND no money in the meantime.)2
