Who to fit bathroom fan?

A good electrician all day long would do that or have a buddy that would knock hole in wall. Their hands are very strong.
A fan fitted directly on outside wall if poss is the best. However, if you have a big bathroom and shower is away from wall then a ceiling one with good ducting

In what way is a through-the-wall fan best? It's pretty easy to find 100mm inline fans which will shift 300m³ of air per hour, and not easy to find a wall fan of the same size which will beat 100m³!1

I can't see how there's much to choose from between wall and ceiling positioning.
'Wall' will be a much shorter air path, so theoretically more effective for the same fan rated extraction figure. 'Ceiling' will look better (Ie less visible), might extract more effectively from the pov that it's drawing from the very top of the room where it's hottest/steamiest, and is presumably more positionable - so could be placed closer to the shower cubicle, for example.
In the OP's case, it would be insanity to fit a wall type when all the ducting is already in place in the ceiling; all that should be needed is a powerful inline fan fitted into the existing duct. It's even possible to go 5" and insert it using adaptors; ok, it might not give the full unfettered 5" performance, but it'll be better than most 4"ers.
NUTS to go wall. Spend what would be a SIGNIFICANT installation cost for core drilling and new power cable on a high quality inliner instead.1 -
