Paypal - item not received case

I opened a case with Paypal for a parcel that arrived with just one item (I ordered two). They kept asking for more information all I could tell them was the same thing again and again, it has been a month! Today I get a message saying to fill this form out and send it back, I have never before have I been required to do something like this - and as for "penalty of perjury" - well, not only do I not know what this means, but also, why would PayPal accuse me of such a thing? I have sent it back anyway, but it is very weird. Has anyone else had this?
