Paypal - item not received case

I opened a case with Paypal for a parcel that arrived with just one item (I ordered two). They kept asking for more information all I could tell them was the same thing again and again, it has been a month! Today I get a message saying to fill this form out and send it back, I have never before have I been required to do something like this - and as for "penalty of perjury" - well, not only do I not know what this means, but also, why would PayPal accuse me of such a thing? I have sent it back anyway, but it is very weird. Has anyone else had this?

They have not accused you of anything. They are asking whether its truthful.

Unless they call your statement a lie then no accusations have taken place.
