Thomas Cook package booking with Ryanair flights

My Wife has booked a package holiday with Thomas Cook for the October half term with two work colleagues. Flights are with Ryanair. She has been now told not to fly due to a medical issue pending further investigations.
They now want to move to October next year. On contacting Thomas Cook, they are happy to rearrange the hotel, but have told her she needs to change the flights direct with Ryanair, for which they want there is a charge £90 return. The £90 can be reclaimed on travel Insurance. Only for one person only.
Would it be correct changing flights direct with Ryanair if the package is booked with Thomas Cook?

“4.14 We will usually provide you with the Booking Confirmation for your flight (and can assist with changes) but there may be certain circumstances where the airline Supplier may provide a Booking Confirmation directly to you, which will mean that we will be unable to access or make changes to your booking and any changes to your booking must be made by you, directly with the airline Supplier.”1 -
