Passport Renewal - Name Change Documents....

Hi all, 

I have just opted to renew my passport online, and I have submitted the appropriate information. I am now at the stage where I need to send over the requested documents. I have read the list of options, however I have very little from within the past 12 months on paper as everything is paperless. I do have clinic letters from the hospital after treatment, and I have a letter from Ikano Bank as I am paying off sofas via them on behalf of DFS. 

Would either of these do? Not sure if a clinic letter would fall under the NHS medical card, or if the DFS loan breakdown would be similar to a bank/utility.

  • Just to add, I have just found my Medical Exemption Certificate which is within the date too. Would this be okay (if none of the others are?)
  • Has your name changed since your last passport ? 

    If it’s a simple renewal in the same name you only need to send your old passport and new photos . 

    If your name has changed - a bank statement can be used - print out an electronic one and send that . 


  • I need name change. 

    I have done now, I am just working 6 days inclusive of today. I was hoping to have my OH have it sent off today. Now it’ll have to wait until Saturday. Was hoping I had something sufficient lying around already.

    if its a change of name by marriage  all you need is a marriage certificate.. For all other changes of name (change of name deed / statutory declaration)  your driving licence or letter from any other government department will do.

    You assume the OP has these documents and they are in her new name.

    It is a change of name from a divorce. So I needed to have a letter from within the past 12 months that reflect this. Everything I have is paperless, and the few things that I do have were either not suitable or out of date. As I mentioned above, I was hoping to get my hands on something Saturday evening to send everything off on Sunday. This didn't happen and I printed off a bank statement on Sunday and then I was very fortunate to get it sent off this am. It was delayed by a day, but I did think that it would be held up by six days.

    Are you reverting to your maiden name?  if so you need your birth certificate, marriage certificate, divorce document (if you have one), a statement saying you are reverting to your maiden name and evidence of use of your new name.

    Are you reverting to your maiden name?  if so you need your birth certificate, marriage certificate, divorce document (if you have one), a statement saying you are reverting to your maiden name and evidence of use of your new name.
    According to the Passport Office page on the "What to send" Page. 
    (It was all sent off Monday following this)

    Applicant’s documents

    Send the following:

    • old passport
    • full birth or adoption certificate
    • marriage or civil partnership certificate (if you don’t have the original you need to get a replacement)
    • a signed and dated letter saying that you use your previous name for all purposes

    And 1 of the following from the last 12 months, that shows you’re using your new name:

    • utility bill
    • bank statement
    • letter from your employer
    • driving licence
    • medical card
    • letter from your local council or a government department

    If any of the following apply, you need to send additional documents(opens in new tab):

    • include a professional, or other title on your passport
    • apply for a British National Overseas (BNO) passport
    • apply for a British Protected Person passport
    • change your gender on your passport
    • renew a passport that was issued for 1 year or less
    • change British nationality status in your passport to British citizen
