Scotland are miles ahead of England in terms of eyecare. Their optometrists are paid significantly more by the NHS but more is expected.

In England for routine stuff like cataracts the hospital will not get paid unless they have a referral from the GP ( or sometimes a triage service ) so that is the real limiting factor. The GP tends to give a medical summary which is useful but can easily be obtained by the hospital at a later date.

Different NHS trusts have different policies
Are you sure any of them have a policy to refer a patient from someone who has eye testing equipment to someone who has no eye testing equipment, so they can refer them to someone else who does have eye testing equipment?
That is what happened with ny husband so yes, it happened.

Just for the record, we have a satisfactory outcome.  Vision express have refunded our £700, and once my wife has seen the orthoptist chappie in November, we go back to them for two new pairs made to the orthoptist's prescription.
    We're all doomed

    Well that is good news.

However, when the time comes I would seriously shop around as their prices seem high!
