There is a question on the form:

    Number of entries requested:

    Single entry
    Two entries
    Multiple entries

I'm not sure which one to tick. Presumably on your suggestions it would be Single entry?

If she is only planning one visit to Malta (or other Schengen countries) then single-entry is the most appropriate. And of course it depends on her travel history: if she has made several trips to countries that attract illegal immigrants and returned on time then she might be trusted with a multiple-entry visa.
She's never left the Philipinnes, unlike "foreigners" (anyone who isn't a Filipino) who have to leave once every year for a day to enable them to renew their visas.

There is also a Catch 22 senario. She is supposed to provide evidence of travel/health insurance to all Schengen countries but travel agencies won't sell this until the flights are booked. I don't want to do this in case the visa application doesn't go through.
You do not need to buy travel insurance from a travel agent (and normally that is an expensive way to go about things). You/she can buy travel insurance (including health) at any time from True Traveller or Nomads, although these two companies are more expensive than most. Otherwise she should see what insurance is available in Manila: probably best to ask in an insurance company or bank rather than a travel agent. Check carefully what cancellation terms are in case the visa is declined.

Realistically, unless she has a good job and so could realistically afford to buy the flights for herself, she is unlikely to be given the visa.

(Being nosey, but did you choose Malta because it is very easy to get married there?)

Well, when I lived in the Philippines I had no need to leave, but I had a proper employment visa. And many filipinos never travel to foreign countries because the travel tax is so high.

Or alternatively try to find an airline who will offer refunds in the event of visa refusal.

A bit of a difficult point at the moment (plus that they don't fly either to Philippines or Malta), but Ukraine International normally have a guarantee of this nature, so I know they do exist. They're normally one of the cheaper airlines for getting from Europe to Thailand etc, so it doesn't have to cost over the odds.

Nope. I find it very hard to learn languages. So I choose countries with English as their second language. Kenya, Philippines and now Gozo.

In most of Eastern Europe, many people hnser about 40 speak English perfectly. Think Poland, Romania etc.

I speak some of the language of both but not enough to do everything required, so English comes as fallback language for me without problem.

Failing that Russian often works, but even a fallback of Google Translate (which yes I have used in the past to hold a basic conversation) works.
