I believe not really - unless it is positioned somewhere that needs assessment for if it is getting dangerous.  Only thing I know to do is general support for the tree health - consider watering if there is a drought maybe, but it is big enough it probably has the roots to cope.

It looks diseased and suffering from die-back. Probably has a bit of canker too. In the long term the tree will most likely die and there isn't really much you can do to prevent it. the canker will spread to other pear & apple trees close by, so planting a similar tree near by isn't a good idea.

That looks like it's ready to fall, near a boundary too.
Shame as oldd trees often give great flavoured fruit.

    That does look better.
That looks like a fungus. Perhaps there's a way to treat/kill/slow it down.
What about getting a hefty stake/prop put in like they do for old oak and yew trees in public places. An independent fencing guy should be able to say and give a price.
I kept a rare plum tree going for years. It had honey fungus that didn't spread to other plants, the branches eventually fell off one by one and when cut to about 5ft from the ground sprouted again :-)
I wouldn't take the big branch out as it seems to be balancing the lean by it's weight - but that's only from photos.
Although it is so well established it wouldn't hurt to feed it. If you ask on the Gardening Forum I'm sure someone will know how to do that.
Lovely bark on it, beautiful patterning.
