    Its more likely a rogue employee in an environment with poor systems of control than a systemic issue or an active decision from the organisation.

    That said, some GPs at least are fairly blasé about these things... took my GP surgery 2 years to update my address properly with them sending the occasional letter to a strange hybrid of my past and present address (thankfully just a few buildings distance between). Reception kept saying they'd corrected it, kept reverting back, next receptionist insisted it must have been a failing on the last person rather than an IT problem.

    What garage? Police took my bike (bicycle) back to the neighbours and I went to A&E

    The only people who had my number were the hospital and obviously the other person's insurance (later on, who I couldn't even speak to until their driver admitted it to them) who weren't going to sell that to third party accident firms - the first thing they said was to offer me a new bike!
