Please Help: A few questions regarding reapplying for licence (1 year abstinence)

Dear All,
First and foremost I hope all is well and safe.
I really would appreciate your input and help regarding my reapplication for my licence.
I bit of a background:
Last year the DVLA informed me on 02 July that they were revoking my licence because I showed a history of alcohol dependence. In April- May last year and the CDT result came back at 0.6. Prior to this I I had declared that I was prescribed a medication that could affect my driving - not alcohol related. After going backwards and forwards this has been going on since 2020. The DVLA wrote back to me saying I need a medical. So I obliged. Prior to this I did not know that I needed to show any abstinence. On the questionnaire I stated I had a drink in Feb.
This year in Jan I took the initiative to get a full medical and CTD test with my GP - the surgery is the same as where I'd taken the previous medical. Now, having been abstinent for over a year (from Feb last year), due to Covid, these two tests are the only ones I've had, I could not do any other testing due to the surgery not having blood bottles and restrictions. The results I got back again were CTD levels 0.6 (no surprise) Can I kindly ask If you can help in answering my questions below.

- Would be okay for me to now reapply for my licence with what medical information I currently have?
- Would the 1 year of abstinence count from the last drink I've had (Feb) or from the date I received the DVLA telling me my licence was revoked, which was in July.
- I got in contact with the DVLA and asked for the D1 form, but I'm absolutely stumped as to which medical questionnaire to fill out? I have been sent a DG1, DR1, M1 and FEP1. Do I need to fill out every single one?
- Also, do I have to pay a fee for reapplying? On the form it says a revocation fee of £50. My licence was not revoked because of any drink driving related incident, so does this fee apply.
I really appreciate all the help you fine MSE folk can offer on this matter.
