DMHEF Mental health Debt write off - Does Anybody have a (un)successful story ? Please share :))

Hi Guys, I am on a DMP and have a history of Mental Illness. I have been doing my Lurk work and I have seen so many people suggesting that people forward there situation with evidence to their creditors and some will write their debt off.
I wanted to know has anybody had debt actually written off? for all the suggestions I have missed anyone saying they actually completed this process.
So please share if you did, I currently am awaiting my clinician to return my forms before forwarding to CAB to forward.
If you have been through this process and it went for you or against you please share, I am interested in knowing if it changes anything if they reject you, or if anyone got accepted or what the actual process is.. its pretty daunting sending these dubious debt collectors medical info...I really dont want to be on an eternal token payment treadmill either - I just got a letter from a creditor saying my last payment is in 2276 so..
