quick question, would having my card amex card on my paypal acc as the only card, would it be classed as a cash withdraw if i send cash to somone using paypal/amex
or would i have to add a debit card and use that

Whilst PayPal causes some challenges with S75/chargebacks etc they are ultimately a payment processor and paying a third party with a credit card on PayPal shouldn't count as a cashlike transaction. If you use "friends and family" then the recipient avoids the card processing fees and certainly some have raised the question of if credit card providers treat these differently to "purchases"... my experience with AmEx is that they don't.0

I can't comment for Amex, but I've used a few different Visa and MasterCard credit cards to send money to "Friends and Family" via PayPal, and these have never been treated as cash transactions, and neither me nor the recipient have been charged any fees or interest by either PayPal or the card company.
