    I'd imagine that ply or similar was added to the frame shown.
    MJB, it is incredibly unfortunate for you to have had the new shower also fail, but this isn't your fault. So it would appear to be 'just another' insurance claim situation.
    If their eyebrows are raised, then you simply explain the situation as you have done here. Once their assessor comes out, it'll be up to them whether to try and claim off the installer, but I doubt they will - unless, perhaps, they find an obvious example* of an incorrect installation when they rip out the existing. But that framework looks to me like the work of a person who is carrying out a good, solid job.

    *Eg, in the very unlikely event that no sheet was first placed on the that frame, or they used something clearly unsuitable. I doubt they will.

    I'd tend to agree with bendy house.
    It's very similar to a frame i built myself (as a diyer) and beyond i used ply on top (as per the destructions that came with the tray). It looks almost identical.

    So assuming ply and correct bedding was done the only reason for failure, in my eyes, would be manufacturing defect or something getting dropped?

    Either way - let the assessor deal with it and just explain it as you've done here. :)
